- Osler coined the term ( to denote appearance like "fresh fungal vegetation" )
- Can be Fungal or Bacterial , majority Bacterial
- Can be seen in Cerebral or Systemic circulation
- Due to occlusion of vasa vasorum by tiny septic emboli
